The second single from the sixth (fifth in the US) studio album Unbreakable by American vocal group Backstreet Boys was released on October 30, 2007. The album included the song Helpless When She Smiles. John Shanks produced the song, which was made available on December 26, 2007. In May of 2007, both this song and Happily Never After, which was never included on the CD, were leaked online.

The song was composed in 2004 specifically for Rascal Flatts. It was later recorded in 2005 by Dutch artist Bastiaan Ragas, in 2006 by German singer Mike Leon Grosch, and in 2007 by the Backstreet Boys.

On November 13, 2007, the video for Helpless When She Smiles was shot in Joshua Tree National Park in California. The filmmaker is Bernard Gourley. Yahoo! song debuted the song video on December 12. The group is shown standing in a large corn field in the black-and-white version of the video, which was originally shot in colour. Similar to the Backstreet Boys video More than That, it has helicopter shots. On the Much More Music Top 10 Countdown, the video reached at number one.
